Login Information:
VIN: |
Your six-digit file number (use leading zeroes, if necessary)
Example: If your file number is 5555, then your VIN is 005555. |

Your 8-digit birthdate (MMDDYYYY)
Example: If your birth date is March 1, 1950, then your PIN is 03011950. |
Web Survey Instructions:
- Go to www.unitedafa.org and click on the “Contract Negotiations Survey” link. Enter your VIN and PIN, then click “Login”.
- You will now be required to change your PIN to ensure the privacy of your survey responses. Change your PIN to any 4-8 digit number you choose. You must remember your new PIN, because you will need it if you wish to access the survey system again.
- Once you've changed your PIN, type your new PIN in the login area.
- From the menu on the left side of the screen, select “Take Surveys.” Select “United Negotiations Survey 2009” from the pull-down list. Then, click “Request Survey”.
- Mark your response to each survey question. Submit finished survey by clicking “Cast Survey.”
- Write down your confirmation number – this is your survey receipt – and then click “Logout.”
Save Responses and Avoid Time Out
If you do not click on a selection item or type a response in a response box within 30 minutes while on the survey screen, the survey will “time out” and you will need to log in again. If you need to step away from the survey for some reason, it’s better to first select “Save Survey,” and Logout. When you return to the survey, sign in again and select “Resume Saved Survey,” to access your previously saved responses. Do not select “Take Survey,” as this will restart your survey responses from the beginning.
If you select the option to “Save Partially Completed Survey,” you will be advised that your responses will not be submitted as part of the survey tally until you click the button to “Submit Survey Responses.”
You may start responding to the survey and return to complete your responses at a later time. However, a suspended web survey may only be resumed on the web; similarly, a suspended telephone survey may only be resumed by telephone.
Technical Assistance
After you complete the survey, you may change your choices, through either the Internet or the telephone method. To do so, you must use your new PIN to access the system again. Only the last survey response will be received. If you experience any problems with the survey system, please contact the Survey Help-Line at 1-800-624-3907, Ext. 546.
More Feedback
For either internet or phone open ended questions, there is a maximum of 1,000 characters. Should you wish to add more comments, please go to our website, www.unitedafa.org, and e-mail your comments via survey@unitedafa.org.
Once you have completed the on-line survey, encourage your flying partners to take part in the survey by Tuesday, January 27th at 5:00 pm Central Time.
Telephone Survey Instructions:
- Dial 1-800-706-7726. Entering your responses on-line is preferred since the length of the survey makes telephonic response difficult. International based Flight Attendants will have the option of using the toll-free International AFA numbers to connect to the 800-706-7726 phone line. These calls may be placed by international based Flight Attendants from any of the following cities: Frankfurt, 0-800-1015247; Hong Kong, 800-964-443; London, 0800 917 3126; Narita/Tokyo 00531-121081.
- Once you reach the survey phone line, you will be prompted for your VIN and PIN.
- The phone system will guide you through the process of changing your PIN, selecting the survey, and choosing your answers. Remember PIN to access system again.
- Use the touch-tone keypad on your telephone to enter your responses to respond to the spoken questions. Open-ended responses or comments will offer you choices of “No Comment” and “Comment”. If you wish to record a response, select “Comment.”
- Once survey is completed you will receive a confirmation number to be saved as your receipt.
